CAPSH works to help low-income families and individuals residing in one of the County paid shelters with transitioning from shelter to safe and affordable housing.
CAPSH’s Homeless services help:
- Individuals
- Youth (ages 16-24)
- Families
- Veterans
- Ex-offenders
Clients must be residing in a county paid shelter and have a Hennepin County referral in order to receive case-management and re-housing services (this excludes CAPSH’S Transitions Services) .
CAPSH’s case managers assist clients with:
- Locating and securing housing
- Landlord/tenant mediation
- Application fees
- Rent subsidies
Case management services include weekly phone calls, monthly home visits, and referrals to resources to assist clients in maintaining their home and basic needs (food, healthcare, employment referrals, etc. ) to achieve housing stabilization and self-sufficiency.
