Community Clinics

CAPSH works with the Park Nicollet Foundation and the congregational nurses program to bolster the health care needs of low-income and vulnerable people in suburban Hennepin County. These Park Nicollet-sponsored community clinics offer health care services to children up to eighteen years old who are part of their respective school districts. These clinics were established based on the need for affordable access to medical treatment. Call 651-690-7029 for appointments, eligibility requirements and program information. The seven participating clinics are:

Walk-in visitors are welcome at the Wayzata location. The Creekside location also accepts appointments for Central Clinic after hours.
